THAT people undergoing medical procedures should give their informed consent might seem simple and uncontentious. But what if a patient has a mental impairment and his doctor does not have time to ensure he understands the proposed treatment? Those who try to look after the interests of such people say that, in practice, hard-pressed hospital staff often ask leading questions and the “consent” obtained is thus far from informed.

A team of researchers led by Suzanne Conboy-Hill, a psychologist at the Royal Sussex County Hospital in Brighton, England, reckon virtual environments could provide the solution. They are designing a virtual model of the hospital and, in October, it will receive its first “patients”—a group of 20 volunteers with learning disabilities who will visit it in order to find out what’s what and, in particular, to be talked through the sort of treatment they might be offered if they really were patients.

The hospital is being built in Second Life, an online world in which people participate in the form of virtual representatives known as avatars. In the case of those in the study, their digital alter egos will begin their journeys at a simulation of the Grace Eyre Foundation centre for adults with learning disabilities in Brighton, which they attend in real life. Local landmarks such as the town’s famous seaside pier will be used to help familiarise them with their surroundings before they arrive at a virtual version of the Royal Sussex.

This particular version of Brighton will, however, be based in London. It will be hosted in a private part of Imperial College’s virtual postgraduate medical school in SciLands, a dedicated science and technology Second Life continent populated by around 30 universities, government agencies and museums. Once inside the hospital, the avatars will be directed to a waiting room. Then a virtual nurse will take them to a bed, where they will lie down to have virtual blood taken. A Second Life doctor will explain that they are about to have an anaesthetic. Meanwhile, a real person alongside the participant will answer any questions and, after the virtual visit is over, Dr Conboy-Hill will carry out “non-directive” interviews, a technique used by the police to encourage witnesses to choose what they want to talk about and to maximise recall.

If these interviews suggest the participants understood what was going on well enough to give informed consent, then the researchers plan to carry out a larger study, comparing their simulation directly with the standard method of obtaining consent face to face. If it proves better, who knows, it might not only be the mentally impaired who find that virtual doctors are better at explaining things than real ones.

For my college courses, I use Articulate Presenter, Crossword Compiler, and Jeopardy to engage the students in learning with games, as well as to provide slides with audio. The web 2.0 tools besides a learning management system (Blackboard, Angel) include Google Calendar, Docs, Talk, Sites, Video Chat, Blogger, del.icio.us, Twitter, MySpace, Facebook, YouTube, Skype, ooVoo, Camtasia and others.

The virtual world of Second Life is extensively used for constructivism activities using Archipelis and tools in world. Community in the online world is encouraged with avatar creation, in world classes, and field trips to events in Second Life. Animations are imported into Second Life with Poser, sounds with Audacity, and images with Gimp or Photoshop. Students interact with virtual patients, making critical decisions for assessment, communication and treatment. Support groups in Second Life are numerous and interaction with them is critical for understanding the complexities for those accessing the medical system with major and long term health issues. Virtual Ability and SL Healthy have more information about these activities.

My teaching career has spanned 27 years in nursing, with more than eight years teaching online. For five years I was the faculty trainer for all disciplines in online teaching at a college.

AMIA Panel Discussion on Virtual Worlds Takes Place in RL and SL

On Wednesday, Nov. 12, 2008 a panel session of the American Medical Informatics Association (AMIA) was held simultaneously in rl and sl. The title of the session was “A Virtual World as a Healthcare Information Platform” and the participants included moderator DanielS Cisco Systems (rl Danny Sands from Cisco Systems), Pathfinder Linden (rl John Lester, Linden Lab), Cooksbranch Chowderhead (rl Daniel Hoch, Massachussetts General Hospital), Deep Semaphore (rl Rameshsharma Ramloll, Idaho State University), Lancelot Spitteler (rl James Kinross, Imperial College London) and Davee Commerce ( rl David Taylor, Imperial College London).
DanielS Cisco Systems began the session by introducing the panel members and giving an overview of the potential ways that Second Life can be used by those in the business, healthcare and social science fields. Cisco sponsored this panel discussion; the first of its kind (discussing virtual worlds in rl and sl simultaneously) at AMIA. Previously, Cisco was involved in another sl/rl presentation, this time at the Healthcare Information and Management Systems Society (HIMSS) meetings of February, 2008, when the Virtual Palomar Hospital sim was introduced. Pathfinder Linden was the first panel member to speak. He is Linden Lab’s Boston Operations Director, having joined Linden Lab in 2005, bringing experience in online community building, healthcare and education to his position. He described virtual worlds and explained that the great challenge is to help people discover the potential uses of this new technology, especially in the areas mentioned.
Cooksbranch Chowderhead (rl Daniel Hoch) was the next to make a presentation. Hoch is a neurologist at the Massachusetts General Hospital who has been working on a study in which Second Life is used as an alternate way to provide healthcare delivery. A technique already taught in rl was chosen for the study, the Relaxation Response, a stress reduction therapy first described by Dr. Herbert Benson and designed by the Benson-Henry Institute for Mind Body Medicine. The teaching of the Relaxation Response was adapted for Second Life and three groups of participants followed the eight week course. Two of the groups have recently completed the program and the results are being evaluated.
Following Cooksbranch Chowderhead , Deep Semaphore (rl Rameshsharma Ramloll of Idaho State University) showed a video of a simulation for emergency training (the title is “Exploring Emergency Preparedness Training in a Virtual Environment: Highlights from a Pandemic Influenza Triage Exercise”) that was done in Second Life. ( http://www.youtube.com/watch?v=v1doVBFl9fs, also see http://play2train.org) The immersive quality of Second Life lends itself to doing this kind of work. As well the organization of these kinds of exercises, which involve large numbers of people working in different disciplines, can be easier than in rl.
The final speaker was Lancelot Spitteler (rl James Kinross) from Imperial College London. Imperial College London, along with the National Physical Laboratory, have created a virtual hospital in Second Life (the Second Health Hospital) in accordance with “Healthcare for London: A Framework for Action”. This hospital allows people to experience the recommendations set forth in “Healthcare for London: A Framework for Action” in a tangible way. Visitors also have the opportunity to leave comments that could be utilized in the creation of rl healthcare services in London. Imperial College London also has a training hospital in Second Life where students can learn to examine patients in a respiratory care unit.
James Kinross spoke about how Second Life can and has been used so far. He talked about both the illusion of interaction that can be felt in Second Life and that these interactions can be captured and analyzed. Kinross’s avatar Lancelot Spitteler actually got up out of his seat on the panel and entered into the audience area, greeting avatars who were seated there and hopefully demonstrating to the session participants in rl just how real the feeling in sl can be. Convincing people who have not been in Second Life of the intensity and reality of the experience is probably one of the most difficult things to do.
Finally Davee Commerce, also of Imperial College Hospital, took the audience on a tour of an operating room which had been set up to help students learn procedures, such as scrubbing up. It was an interesting experience because many of the objects in the room were set up to warn you if you were doing something wrong. For instance, I bumped into an object in the room, a cart, I think, and was told by the object that I had done so.
This AMIA panel discussion illustrated many different aspects of the use of Second Life. The actual presentation itself was made available to those who could not attend the meeting in person. Many different healthcare related uses were also demonstrated: teaching patients who might not be able to get to the hospital easily (demonstrated by the teaching of the Relaxation Response), training of large numbers of healthcare professionals and emergency responders to react to emergency situations ( the video shown by Ramesh Ramloll on the emergency response to the flu epidemic), the ability to help form healthcare policy (Second Health Hospital) and training of medical students (Imperial College London). The use of virtual worlds such as Second Life can help save healthcare systems time and money, something of great importance in this period of increasingly costly healthcare. for a description of a visit to the virtual hospital)

The medical community is trying to find real-life benefits from the virtual reality of Second Life.

In Second Life, a computer world where just about anything goes, several physicians and health care professionals are finding that “anything” can include work that makes real life easier for themselves, their colleagues and their patients.

But in a world void of the laws and protections governing real-life medical professionals, taking advantage of the opportunities a virtual world offers comes with risks.

“There’s more to it than ‘Hey, this is a very cool application,’ ” said Maurice Ramirez, DO, PhD, president and founder of the Orlando, Fla., consulting firm High Alert. “Done right, this could solve a myriad of problems. Done wrong, it is a security nightmare to not only patients but to doctors.”

Reporter Pamela Lewis Dolan crossed between real life and virtual life for this story on the medical-related activity in the online 3-D world inhabited by millions since San Francisco-based Linden Lab launched Second Life in 2003 (www.secondlife.com).

Passages in italics indicate Dolan is navigating Second Life as Annastasia Blaisdale, her avatar, or online character. Straight type represents Dolan writing as herself. Grand Opening of Virtual Ability Island, in Second Life

VIRTUAL ABILITY ISLAND GRAND OPENING!

Join us on Saturday, August 16th at 1 PM SLT to celebrate the Grand Opening of Virtual Ability Island.
The island was developed with a new resident orientation center, advanced tutorials, and training facilities for individuals with disabilities or chronic health conditions. The entire island is accessibility-friendly for new and experienced residents and will provide ongoing classes for people who want to search for and evaluate health information. Funding came through an initial grant from the National Library of Medicine to the Alliance Library System and is sustained by the generosity of individual contributors to Virtual Ability, Inc. We are excited to present this important resource to members of the Second Life community!

WASHINGTON - Dopo Google Map, arriva HealthMap un nuovo servizio interattivo disponibile su Internet che mostra la diffusione delle epidemie. Si tratta di una mappa interattiva, creata da Google e da un gruppo di ricercatori del Boston Children's Hospital e dell'Università di Harvard. Il sistema è accessibile a tutti, è gratuito ed è stato descritto sulla prestigiosa rivista scientifica «opena access» Plos Medicine. HealthMap è stato creato con lo scopo di monitorare le emergenze e facilitarne la gestione.

LE FONTI - Il sistema raccoglie i dati da 14 fonti (che riassumono informazioni da più di 20 mila siti) ed è in contatto diretto con l'Organizzazione Mondiale della Sanità (Oms). La ricerca può essere effettuata per malattia, oppure per sintomo e aiutandosi con alcune parole chiave. Un software provvede anche ad eliminare automaticamente gli articoli con informazioni i ridondanti.

UTILITÀ- «Il Web può giocare un ruolo molto importante nella scoperta di nuove epidemie o nel monitoraggio di quelle già in corso», ha detto John Brownstein, responsabile del progetto. «Molte informazioni utili - ha continuato - possono essere ricavate dalla rete grazie a siti governativi, ma anche forum e blog». Da un primo utilizzo è risultato evidente come la frequenza di dati riguardanti alcune malattie sia direttamente proporzionale a una situazione economica instabile. Si sta cercando ora di migliorare il sistema in termini di copertura geografica, che per il momento arriva solo a livello di regione e stato. Il motore, inoltre, monitora e organizza i dati in tempo reale anche in zone geografiche poco coperte dai sistemi sanitari locali. Sarà però necessario sviluppare contatti con persone residenti nei luoghi meno 'nominatì in Rete, soprattutto in Africa e Sud America.

About HealthMap

HealthMap brings together disparate data sources to achieve a unified and comprehensive view of the current global state of infectious diseases and their effect on human and animal health. This freely available Web site integrates outbreak data of varying reliability, ranging from news sources (such as Google News) to curated personal accounts (such as ProMED) to validated official alerts (such as World Health Organization). Through an automated text processing system, the data is aggregated by disease and displayed by location for user-friendly access to the original alert. HealthMap provides a jumping-off point for real-time information on emerging infectious diseases and has particular interest for public health officials and international travelers.

Second Life offers healing. Therapeutic options for users

… A lot of altruism, free giving, plenty of warnings. Isn’t there any digital snake oil here? No fake therapists?

One in-world psychologist, Dr. Craig Kerley from Georgia, who was profiled on CBS’s “Early Show,” has hung his shingle for “cybertherapy” at $90 per hour. This work, he says, “can be valuable for those who have limited choices in their geographical region, have limited time to drive to regular in-person appointments, have limited mobility, and have limitations in their lifestyle that make traveling to a brick and mortar office difficult.”

Still, Dr. Peter Yellowlees, a professor of psychiatry and behavioral sciences at UC Davis and a specialist in virtual worlds, cautions about therapy in Second Life, even with professionals. He advises using it only as “a potential adjunct to face-to-face therapy,” and to “use passwords or other cues in Second Life to make sure you’re talking to the right person” - the real therapist, not scammers posing as one.

Yellowlees uses Second Life as a teaching tool, not for therapy. His Virtual Hallucinations sim gives “the lived experience of schizophrenia - to hear voices and see visions” so his students (and the rest of us) can “get inside the head, just a bit, of someone who’s psychotic.”

It certainly sparked empathy in me, much more richly than a mere clinical description of the disorder would have done.

Empathy: There’s that word again, an odd one to associate with impersonal bytes and modems, but the right one. Second Life is a hot, humming thing of wire and light, a “server” - spiritual teachers would like the metaphor - that can carry community and genuine human sympathy…

Con Google Health la cartella clinica va online. Anche in Italia

Ves Dimov è un medico della clinica di Cleveland, negli Stati Uniti, e autore di un blog molto letto, Clinical cases and images. Anche il suo ospedale collabora con il progetto Google Health: si tratta di una cartella clinica compilata su internet direttamente dai pazienti. Che in questo modo possono affidare a un archivio informatico i loro dati sanitari: allergie, patologie, farmaci in uso, vaccinazioni. Perfino interventi chirurgici e risultati delle analisi mediche. Dopo due mesi di sperimentazione nella clinica di Cleveland un gruppo di persone ha notato alcuni vantaggi: la cartella può essere letta da qualsiasi luogo (anche in situazioni d’emergenza) e si può integrare, su esplicita richiesta, con quella già fornita da alcune strutture sanitarie. Le informazioni sono inviate con un protocollo cifrato e custodite sui server di “big G”. Inoltre Google Health riunisce alcuni servizi già disponibili negli Usa, come l’avviso attraverso sms sul cellulare per ricordare di assumere un farmaco durante la giornata. Eppure si tratta di notizie “sensibili” sulla salute personale: i difensori dei pazienti e della privacy ricordano sul Boston Globe che negli ultimi anni le fughe di dati sono diventate più comuni. Google Health è stato progettato per gli Stati Uniti, ma la maggior parte delle funzioni è già accessibile (in inglese) ai navigatori italiani.

È in rapida crescita l’interesse per le cartelle sanitarie archiviate online dai pazienti : il portale medico più letto negli Stati Uniti, WebMD, consente da tempo agli utenti di compilarle. Anche dall’estero, Italia inclusa. E otto mesi fa Microsoft ha lanciato Healthvault, un’iniziativa per raccogliere sul web le informazioni riguardanti la propria salute, disponibile unicamente per gli abitanti degli Usa.

La ricerca di notizie su malattie e sintomi attraverso internet è ormai un’abitudine per milioni di persone. Secondo la società di analisi comScore, negli Stati Uniti un navigatore su tre si è informato su argomenti sanitari consultando le pagine web. Le parole più richieste? Esclusi i nomi di farmaci, nei primi quattro mesi del 2007 il vocabolo “gravidanza” è stato scritto sul web 17 milioni di volte, seguito da “cancro” e “influenza”, con 15 milioni di ricerche.

Il ‘Web sociale’ sta contribuendo a rendere più informati e consapevoli pazienti e operatori sanitari. I ‘consumatori di assistenza sanitaria’ utilizzano network sociali, blog personali, wiki, video-sharing e altri format del web 2.0 per avere un supporto emozionale ma anche per gestire meglio le loro condizioni di salute e il loro percorso terapeutico. Lo rivela un rapporto della California HealthCare Foundation. Ormai il movimento che negli Stati Uniti hanno definito “Health 2.0”, Salute 2.0, è una realtà. Un fenomeno culturale e sociale che ha spazzato via la vecchia concezione per cui l’assistenza sanitaria è una cosa che si verifica in una stanza tra un paziente e un medico.

Secondo il rapporto “The wisdom of patients: health care meets online social media” appena pubblicato, la crescente domanda di trasparenza da parte del pubblico guiderà l’evoluzione del web sociale in ambito sanitario. Sta infatti diventando disponibile una serie impressionante di nuovi strumenti informativi e operativi, come i siti che offrono l’archiviazione gratuita di dati clinici on-line, e la tecnologia per consultarli anche tramite un telefono cellulare è ormai quasi alla portata di tutti i pazienti e tutti i professionisti del settore.
